The goods and services shown below are those we can provide
to our clientele. You may choose only these items you desire.
However, any funeral arrangements you select will include a
charge for our basic services and overhead. If legal or other
requirements mean you must buy any items you did not
specifically ask for within this contract, we will explain in
writing on the statement we provide, describing the
cremation/funeral goods and services you selected at time of
need. |
BASIC CREMATION

| BASIC SERVICES OF FUNERAL DIRECTOR: This is a basic
charge for each family we service. It applies to burial or
cremation. It includes consulting with the family to make
necessary arrangements; the shelter of remains; the
preparation and filing of necessary documents, obtaining
necessary authorizations and permits; coordinating with the
crematory, or other third parties. In addition, this fee
includes a proportionate share of our basic overhead costs.

| TRANSPORTATION: Removal of the deceased from place
of death within Sacramento County and Roseville. Outside of
these areas an additional mileage fee of $2.00 per mile will
be applied. |

| REFRIGERATION: A few days will elapse from the time
of death until cremation is completed. This time span
necessitates refrigeration since we do not offer embalming.

goods or services, the responsible party of the decedent’s
survivor who is handling the funeral arrangements is entitled
to receive a copy of any preneed agreement in the possession
of the funeral establishment that has been signed and paid
for, in full or in part, by or on behalf of the
decedent. |
| *Direct Burial. Our charge for an immediate burial
(without ceremony) includes: basic services of funeral
director and staff, removal of remains; refrigeration of
decedent, and local transportation to cemetery. |
